Would a "Shofar" with a side blown mouthpiece (i.e. hole on the side of the shaft near the bottom, but not on the end where it would be straight) be Halakhically acceptable? Sources would be appreciated.

Answer
No, one must blow from the end, and contrarily, to make a hole in the shofar may very well deem the shofar unkosher (Shulchan Aruch Or. Ch. 586).
